Title of Position: Traveller Primary Health Care Project Coordinator
Reporting to: Primary Health Care Project Steering Group. On a day to day basis reporting to the manager
Location: Kildare Traveller Action, Meadow Road , Tully East , Kildare Town.
The individual will work together with the KTA manager and Primary health care team to implement a community work approach across the Primary Health Care for Traveller Project Team (PHCTP) and organisation using a social determinants of health analysis. The aim of the PHCTP is to address Traveller health inequalities as identified in the All Ireland Traveller Health Study (2010) and support the implementation of the National Traveller Health Action Plan. They will work to support the PHCTP team to deliver peer led health education, health promotion, advice, support and empower Travellers to manage the wider social determinants of their own health and wellbeing and promote positive collective health outcomes for Travellers. The Coordinator will work with a team of Traveller Primary Health Care Workers currently 8 who work with the local community across Kildare County. The post is funded through the Eastern Region Traveller Health Unit (THU) on behalf of the Health Service Executive (HSE).
Leadership & Coordination:
- Support the management and coordination of the team of Traveller Primary Health Care Workers (PHCWs) in Kildare Traveller Action
- To further develop the activities KTAs Primary Health Care for Travellers Project. This includes piloting new initiatives to respond to the ongoing and emerging needs of the local Traveller community in Kildare
- To advocate for and facilitate the mainstreaming of Travellers within local health services, through the development of culturally appropriate provision, training and implementation of appropriate codes of practice;
- To write monthly, quarterly and annual reports on the work outputs, impacts and key performance indicators related to the project for KTA manager, THU,Primary Health Care Steering Group and the Eastern Region Traveller Health Unit;
- To work collaboratively across KTAs wider team members to ensure adequate supports and responses referrals are made
- Work with team to ensure adequate Traveller representation from Kildare in the Eastern Regional Traveller Health Network (ERTHN);
- To collect data and qualitative information to monitor and evaluate the processes/outcomes of the PHCTP;
- To ensure that the project is run according to current models of good practice and contractual obligations;
- Work with the KTA team to implement the actions from its strategic plan
- To liaise and work with other relevant statutory and voluntary organisations and agencies as appropriate;
- Reduce the barriers faced by Travellers in accessing public services by supporting the development of culturally appropriate services, providing anti-racism and cultural awareness training to service providers and staff.
- To support Community health peer workers in carrying out their duties.
- To develop annual and quarterly work plans in conjunction with all stakeholders
- To write quarterly and annual reports on the work outputs, impacts and key performance indicators related to the project for the Steering Committee and the Regional THU.
- To collect data and qualitative information to facilitate the ongoing documentation of the process/ outcomes of the PHCTP.
- To support and supervise the delivery of health programmes with and by the Community Health Workers.
- To support the induction of new trainees and the continuous education and training of TCHW's.
- To develop new initiatives to respond to the ongoing and emerging needs of the local Traveller community in partnership with the TCHW's and the Steering Committee.
- Promote awareness, interest and knowledge of health matters through formal and informal education methods.
- To support the ongoing development of Culturally appropriate Health Materials for the PHCTP.
- To monitor and evaluate the work of the Primary Health Care for Travellers Project on an ongoing basis
- To ensure that the project is run according to current models of good practice.
- To liaise and work with other relevant statutory and voluntary organisations and agencies as appropriate to the needs of the PHCTP.
- To advocate for and facilitate the mainstreaming of Traveller's within the local health services, through the development of culturally appropriate provision, training and implementation of appropriate codes of practice.
- Write and submit proposal for funders, if and when necessary.
- To consider the health, safety and welfare of the project staff, in accordance with the Health and Safety at Work Act.
- To perform other relevant duties as may be determined by the The Primary Health Care Project Steering Committee.
- To support the induction of new trainees and the continuous education and training of TCHW’s.
- To support the ongoing development of Culturally appropriate Health Materials for the PHCTP.
- To monitor and evaluate the work of the Primary Health Care for Travellers Project on an ongoing basis
- To develop annual and quarterly work plans in conjunction with all stakeholders towards addressing and improving the health status of the local Traveller Community and responding to existing and emerging health needs.
- To collate statistics and return HSE Key Performance Indicators (KPIs) to the THU and to highlight the role and benefit of the operations of the PHCTP;
- To convene and support the PHC steering group
- Highlight and address the social determinants of Traveller health through supporting the THU roll out of ethnic equality monitoring across health service administration systems;
- To participate in organisational and other meetings where applicable;
- To perform other relevant duties as determined by the Board of Management and manager of KTA.
Qualifications/experience:
- Level 7/8 qualification in social sciences, community development, human rights or other related health associated disciplines; or
- At least 2 years’ experience in community development or human rights work, human rights and/or community project;
or with substantial experience of working in a community setting or with Travellers or other marginalised groups.
Skills & Knowledge:
- Coordination, leadership and management skills;
- Strategic advocacy and campaigning;
- Skills in advocating to challenge racism and promoting equality and human rights;
- Experience in facilitation and group work skills;
- Strong understanding of community development, human rights and social determinants framework;
- Excellent analytical and writing skills;
- Excellent communication and interpersonal skills, including sensitivity and appreciation of intercultural and anti-racist approaches in a culturally diverse environment;
- Ability to work on own and in a team environment
- Show creativity in solving problems;
Other essential requirements
- Holding a clean, full driving license and ability to travel
- Flexibility in working outside business hours if required
Terms & Conditions
The appointment will be for a fixed term contract of 12 months, for 19.5 hours which may be renewable, depending on, and in line with financial allocations from the THU overall direction and development.
