Contract: Full-Time, Permanent contract

The purpose of this role is to work with residents of George’s Hill STA across a range of services internally and externally, with the aim of moving residents out of homelessness and into stable long-term accommodation. The Case Manager works continuously with the same young people when they enter the service until they move on.

What you will do:

  • To work within the framework of the overall objectives and values of Focus Ireland
  • To work within the overall policies and procedures of Focus Ireland, inclusive of Health and Safety Policy and ensure their implementation
  • To work as part of a 7-day rota and support the daily operations of the S.T.A. including reception cover.
  • To establish trusting relationships especially with young people who are homeless or at risk of homelessness and engage them in work that enables them to move to a more stable lifestyle.
  • To carry a designated case load
  • To meet allocated young people regularly with a focus on moving them on from the S.T.A. as soon as possible.
  • To undertake assessments and support planning with young people as the lead agent using a Case Management system, protocols and other tools to assist the young person to successfully progress toward independent living as appropriate to their needs.
  • To support the young person with their welfare rights and entitlements, training/employment, housing, and to access move on accommodation appropriate to their needs.
  • To provide Advice, Information and Guidance as a prevention or in response to being out of home. The case manager will also ensure appropriate referrals to accommodation sustainment services, and other visiting support services appropriate to their needs, to ensure the viability of their long-term tenancy.
  • To develop and maintain professional links with other agencies and services that may offer services to young people. This includes developing links and relationships with housing providers, Local Authorities and private landlords and agents.
  • To accompany and provide support to young people on appointments to other services i.e. doctors, counsellors, court, property viewings etc.
  • To organise and conduct residents’ meetings ensuring their voice is heard on matters relating to their needs in the S.T.A. And to participate in customer involvement activities to develop the welfare and pro-social behaviour of residents.
  • To supervise young people on outside activities and provide support whilst in custody, if required.
  • To be responsible for keeping up to date casework records, daily logs and written assessments. Inputting information on the Focus Ireland Database systems
  • To work in close liaison and co-operation with other teams within Focus Ireland, in particular the Focus Ireland Youth Services, and other statutory and voluntary agencies as appropriate.
  • To attend Team, Section, Agency and external agency’s meetings and training as required
  • To meet regularly for supervision in line with policy
  • To attend Reflective Practice group with your assigned grouping
  • In conjunction with your Line Manager continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner to meet the strategic aims and objectives of the Focus Ireland.
  • In consultation, to undertake any other responsibilities and duties as may be reasonably assigned by the Project Leader, Manager or Director of Services.

What you will need:

  • A Third Level qualification at degree level in social care i.e., youth work, social studies, addiction, psychology etc.
  • 1 year’s direct experience in the social care field working with disadvantaged young people.
  • A strong knowledge of issues affecting young people out-of-home and their impact on young people and their physical, emotional, and psychological development.
  • Relevant experience of dealing with high risk and challenging behaviour.
  • Relevant experience of working in a team which shares a common goal.
  • Ability to cope with and manage lone working including out of hours’ work
  • A working knowledge of services in the homeless sector. (Desirable)
  • An awareness of child protection issues and ways of responding appropriately to any concerns. (Desirable)
  • An awareness of a trauma informed approach to working with vulnerable populations. (Desirable)
