Clonmel Community Resource Centre delivers the Social Prescribing project for Clonmel on behalf of the HSE Slaintecare Health Communities initiative

We are recruiting a suitably qualified and experienced person to continue the valuable work of this project

Social Prescribing Link Worker 
22.5 hrs per week

Temporary post to cover maternity leave from Aug/Sept to Jan /Feb 2026

Applicants must satisfy the criteria regarding Qualifications and Experience as follows:-

Qualifications:

• Level 8 Qualification in a Social, Community, Health related field

Experience:

• 3 Years + employment in a field of Community Development, Health or Social Care
• Must be able to demonstrate working directly with clients in a supportive capacity
• Must be able to demonstrate working collaboratively with service providers & agencies
• Have a working knowledge of reducing health inequalities in all communities and an understanding of the wider determinants of health

Attributes:

• Person centred, empathetic, motivated, can work on own initiative. Be organised, reliable, professional, efficient, excellent oral and written communication skills to include reporting.
