Case Management Worker

Fixed-Term Contract – 12 months
Salary €37,109.46 per annum
Full-Time, 35 hours per week
Monday to Friday 9.00am- 5.00pm
Based in Cork

1. Role

To work as a part of the Depaul Homeless Health Team, providing case management support to homeless people and marginalised groups who have complex support needs in the area of their medical and physical wellbeing to ensure they have access to the appropriate medical care within the context of a case management approach. To ensure that the relevant supports are in place and to work towards positive outcomes for people experiencing homelessness.
This role is to support those who are homeless and living with chronic health conditions in addition to broader social inclusion cohort e.g migrants, traveller community.

2. Key Areas of Responsibility

  • Provision of assertive engagement support services across community, hospital and accommodation services to Inclusion Health clients experiencing homelessness with complex health and/or addiction needs
  • Provide assertive support to Inclusion Health clients from migrant background with complex health and/or addiction needs
  • To support the presenting needs of clients and manage robust, effective working relationships with external agencies and partners of all health services (mental health, community and hospital based) including Mercy University Hospital, outpatient services, community GPs, researchers, Cork City Council/accommodation providers, and other professionals in homeless and drug services.
  • To assist medical professionals and project workers to engage and co-ordinate health issues of clients in crisis or as part of an on-going care plan
  • To assist with crisis management in the community and within the hospital where required
  • In collaboration with the Inclusion Health team, to support and liaise with external agencies on specific projects and service improvement initiatives e.g. research, peer engagement, conferences and
  • presentation, training, policy development
  • To participate in service reviews and evaluations and to implement any changes arising from such work.
  • Attending weekly Multidisciplinary Team meetings (MDT) and team rounds in Mercy University Hospital or any other location/s
  • . Following up with IH patients post discharge from hospital to ensure client agreed care plan is supported
  • Support to inpatients - keyworking support, addiction support and treatment options to inpatients, liaison, accompaniment to appointments while patients are in hospital, collection of money/acting as agent for inpatients
  • To support engagement with and referrals to Peer Support programmes and ensure accompaniment to hostels, respite/ detox, GP and addiction services on discharge to prevent unnecessary readmission and support discharge care planning.
  • To work directly with service users,utilising a case management approach to ensure access to relevant wraparound services needed for the service user’s progression.
  • To advocate and empower service users to overcome various barriers they encounter, liaise with teams from various services and support clients to maintain their present accommodation in collaboration with tenancy sustainment supports
  • To develop and maintain robust, effective working relationships with other agencies, using an interagency approach.
  • To ensure that all service user’s information is recorded and stored as required and in line with Data Protection regulations and Depaul’s Confidentiality Policy.
  • To participate fully in meetings within Depaul and with external services, with the direction and support of the Line Manager.
  • To work creatively and exercise initiative to help the service meet its core objectives and ensure service user’s needs are met.
  • To adhere fully to Organisational Health & Safety Policies, especially in relation to assessing risk and to lone working.
  • To at all times undertake the role in a professional manner, always working in accordance with the aims, values and ethos of the Depaul.
  • To participate fully in the supervision structure (including Induction and Progress reviews, monthly supervisions, JARs).
  • To participate in data collection and/or research in line with the requirements of the funder
  • Undertake any other duties that may be required which are commensurate with the role

The above list is not exhaustive; additional areas of responsibility may be added over time and flexibility to cover for other staff roles is required from time to time.

3. Person Requirements

  • A minimum of 2 years’ experience working in the homeless sector or another related field. At least one year of this working with and co-ordinating support plans for service users with complex needs. (Essential)
  • An understanding of why people become homeless or may be at risk of homelessness and the issues they present with.(Essential)
  • Experience of interagency working within the voluntary and statutory sectors (essential)
  • Experience of lone working or off-site working (Desirable)
  • Experience of working alongside health supports, such as hospitals, methadone provision services, primary care services, medical social work etc
  • A clear understanding of the case management approach
  • Be able to work from a solutions focused, creative approach.
  • Excellent communication skills - both verbal and written and electronic.
  • Ability to be flexible and work in a variety of settings, with a variety of teams.
  • An ability to (and understanding of how to) advocate on behalf of the service users
  • An understanding of the statutory and voluntary sector services available to homeless people with complex needs.
  • Lived experience of homelessness and/or related issues will be taken into special consideration
