Community Connector (Maternity Cover) for Liffey Partnership - The Person
Liffey Area Partnership CLG is seeking to appoint to our team a person with a passion for working with older persons, empowering them to take control of their health and wellbeing by adopting a holistic approach. The position is available on a fixed term basis on a 12 month contract with a 21 hour working week.
The Community Connector will work as part of the wider Health, Inclusion and Community Teams in Liffey Partnership providing a specialist service for a specific cohort of older people in close collaboration with the HSE’s Integrated Care Team for Older Persons.
The successful candidate will be passionate about working with individuals to support behaviour change and promote health and wellbeing. They will have a third level qualification in a related discipline (e.g. Social Care, Health Community Work etc) and substantial experience of providing support to individuals experiencing marginalisation or social exclusion in an individual (1:1) and/or group context.
The successful candidate will have experience of partnership/ collaborative working and of building relationships across a variety of organisations and be experienced in data collection, monitoring and evaluation. The person will have the ability to work effectively as part of a multi-disciplinary team as well as ability to work on own initiative. They will have excellent interpersonal skills, including in written and verbal communication. They will be aware of the social environments of the Cherry Orchard area of Dublin as well as be conscious of the additional requirements associated with working with statutory agencies and the protection of public funds. They will be required to understand accept and promote the social inclusion ethos of the Company as well as its remit to deliver the highest possible standard of service to its clients and the local community.
