Is recruiting a Social Prescribing Link Worker full time (37 hours)
Applicants must satisfy the criteria regarding Qualifications and Experience as follows:-
Qualifications : Level 8 Qualification in a Social, Community, Health related field
- 3 Years + employment in a field of Community Development, Health or Social Care
- Must be able to demonstrate working directly with clients in a supportive capacity
- Must be able to demonstrate working collaboratively with service providers and agencies
- Have a working knowledge of reducing health inequalities in all communities and an understanding of the wider determinants of health
Attributes: Person centred, empathetic, motivated, can work on own initiative and in collaboration with a diverse steering group & service providers, organised, reliable, professional, efficient, excellent oral and written communication skills to include reporting, presenting and facilitation.
The ultimate aim of the social prescribing link worker is to connect people to community groups, organisations and statutory services for practical and emotional support with the overall purpose of improving health and wellbeing and improving social support. Social prescribing link workers support existing groups to be accessible and sustainable and working collaboratively with all local partners, identify gaps and needs regarding particular groups or interests. Referrals to the social prescribing link worker come from various sources, including for example GPs, primary care teams, community mental health teams, community dieticians and older people’s services and the community and voluntary sector organisation as well as self-referral.
Social prescribing can help to strengthen community resilience and personal resilience, and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities. It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.
Clonmel Community Resource Centre is a “not for profit” charitable organisation RCN 20032722 which provides a base for community activity, provides family support, delivers a community based childcare service and responds collaboratively to emerging needs in the community.
The Social Prescribing link worker will be part of a vibrant team who will be committed to the vision and ethos of the organisation for the benefit of the community.

Location: Clonmel Community Resource Centre, Kickham Street, Clonmel, Co Tipperary
Remuneration: HSE scale Grade V
Pension: This post is not pensionable
Contract type: Fixed term to December 2022 in the first instance TBA
Probationary period: 6 months
Annual Leave: 22 days
