Job Title: Community Health Project - Leader

Reporting to: Individual Supports Manager

Duration: 1st December 2020 – September 2021

Salary: €41,000 per annum

Donegal Local Development CLG are now recruiting for the role of a Community Health Leader to deliver this project. The successful candidate will work from a base in County Donegal and will implement the project on the ground in either the North or South Donegal Health Hub and will have overall responsibility for all Hubs.

Duties and Responsibilities:

  • Have overall responsibility for the delivery of the CAWT CoH-Sync project which is made up of the 3 Hubs, a) Donegal North b) Donegal South and c) Monaghan/Cavan
  • Co-ordinate the work of a team of Community Health Facilitators in their hub area to ensure that programme targets are achieved
  • Line Manage the Donegal based Community Health Hub Co-ordinator and Project Administrator
  • Establish a Steering Committee to support the work of the project and ensuring it is part of a project wide cross border network
  • Develop linkages with the local community health groups, projects and service providers
  • Plan, organize and deliver information on the CoH-Sync Project to relevant target groups
  • Manage referrals from other Local Development Company programmes/service providers and communicate with these groups as well as the clients referred
  • Proactively target prospective clients with the aim of securing project participation
  • Develop Personal Health Plans for clients that identify health support needs
  • Provide one-to-one information and guidance in relation to all supports, initiatives and services required by project clients and connect them to relevant agencies to ensure successful achievement of their Personal Health Plan goals.
  • Achieve agreed Key Performance Indicators
  • Publicise the work of the programme to encourage participation by others
  • Maintain all records in the manner required (client files, databases, etc) to record activity, client progression and overall programme outcomes
  • Be aware of the developments in the health sector at local, regional and national level
  • Assist in the mapping of Community Health supports in the Donegal Health Hubs
  • Assist the Community Health Co-ordinator to prepare monthly reports for the DLDC Board and programme funders
  • Assist with monitoring and evaluation of the project
  • Carry out any other function relevant to the position of Community Health Project - Leader as indicated from time to time

Person Specification

The successful candidate will have:

  • 3rd Level Qualification
  • Experience in managing a project is desirable
  • An empathy with target groups members of the programme
  • A high level of understanding of the link between prevention/early intervention in health and well being to prevent the development of long term health conditions.
  • Excellent communication and listening skills
  • The ability to motivate others, influence opinion, inspire trust and develop mutually beneficial relationships
  • The ability to form good working relationships with organisations and communities
  • Good organisational and planning skills, good attention to detail in approach to work, have effective time management skills and an ability to work to tight deadlines
  • Have an ability to work flexibly with DLDC colleagues to ensure efficient delivery of schemes
  • Be capable of supervising compliance with health and safety requirements
  • Be able to set, manage and monitor work schedules and attainment of targets by Community Health Project team
  • Good computer/ICT and record keeping skills
  • Access to appropriate transport to fulfil the requirements of the role as post will involve frequent travel.
  • A track record which demonstrates initiative, personal motivation, an ability to make things happen and achieve outcomes.
  • An ability to use independent judgment and handle confidential material sensitively.
  • Any health work background is welcome but not necessary

Conditions of Work:

  • The Community Health Leader will be required to work 35 hours a week
  • A probationary period of at least six months will apply.
  • This is a Fixed Term contract until 30/09/2021.
