Living Well Programme Coordinator

Location: Cork and Kerry Region - a flexible approach to work location will be considered.

Job Purpose:

Living Well is an evidence-based self-management support programme for people who are living with one or more long-term health conditions or caring for someone with a long-term health condition. The programme supports people to develop skills and confidence to become better self-managers of their own health conditions. Most people living with a long-term health condition want to maintain an independent, healthy, and active life and with good self-management support this can be achieved.

The role of the Living Well Coordinator will be to lead and coordinate the Living Well Programme across the Cork and Kerry Region.

This role is funded through the HSE and working under the guidance/direction of the HSE Self-management Support Coordinator for Chronic Conditions will be an important part of the role.

Hours of work: 25 hours per week

Salary: €47,335 pro rata based on a 37.5 hour working week. This role is aligned to HSE Grade 5 March 2023 scale.
