Intensive Case Manager
1 x Full Time Post, 32 hours per week
Four Day Working Week
The post is based in Sligo town covering counties Sligo and Leitrim.
Salary: €32,876 per annum
Role
To work as a part of a supportive, multi-disciplinary team delivering a high-quality Housing First service in the Northwest of Ireland. Housing First is a model of housing that provides housing with wrap around supports to those who are long- term homeless and who have a history of complex trauma and rough sleeping. The service provides support through intensive case management with multi-disciplinary specialisms to provide support to tenants in their own homes. The team is guided by the ethos and principles of Housing First, to collaboratively provide person centred support to individuals living in their own homes. The Intensive Case Managers (ICMs) are qualified social care workers who manage a case load of individuals with varying needs and intensity of support. Their role is holistic and adapted to the needs of the clients. Day to day supports involve advice and guidance with managing money and bills, support in attending appointments, community integration and life skills. The approaches are based on recovery orientation and are trauma informed lead. The Intensive Case Managers will also be required to advocate on behalf of the service users to ensure that they are adequately represented within the community to have their needs met.
Location
Northwest Housing First is delivered across the region of the Northwest of Ireland, covering Sligo, Leitrim and Donegal. There are offices based in Sligo and Letterkenny town.
This post will be based in Sligo town, covering counties Sligo & Leitrim.
The service is currently provided Monday to Friday 8.30am to 6pm. With the intention of the service being extended to Saturdays in the future. Saturdays will be worked on a staff rotational basis.
Successful applicants will:
- Be a registered with CORU or in the process of registering with CORU.
- Hold a Level 7 degree or above in Social Care or CORU recognised equivalent.
- Have relevant experience in working with marginalised groups.
- Access to own car and a full driving licence.
