
The Role
We have an exciting opportunity for a Project Worker – Rough Sleeper Team whose role is to work closely with the Outreach Services Manager to support the integration process of outreach services, and take the lead on key projects on the Rough Sleeper Team. Implement and carry out effective Care and Case Management with the objective of moving clients into accommodation or residential treatment services following their engagement with the Rough Sleeper Team.
Core responsibilities of the role include:
The project worker will work closely with the Outreach Services Manager and the team and take the lead on key projects, specifically to:
- Drive Care and Case Management processes.
- Provide in-reach and outreach services.
- Act as the key liaison on harm reduction with external and internal stakeholders.
- Induct new team members and carry out relevant on- the-job training and guidance.
- Deliver a quality effective service ensuring the progression and sustainability of all clients.
- Manage a case load and as part of a multi disciplinary team work to achieve service targets.
- Engage clients with appropriate accommodation and support services required as identified in support plan.
- Deliver the Emergency Needle Exchange Programme and other Harm Reduction initiatives.
- Conduct Early Morning Street checks.
- Implement and work within care and case management systems and protocols.
- Perform all the administrative duties relating to the role in order to ensure that clients are supported in an effective and timely manner.
- Work in cooperation and partnership with other agencies, both voluntary and statutory ensuring good communication and cross-agency flexible working practices to minimise obstacles to a person getting access to supports, services and/or accommodation
We are looking for someone with a recognised third level qualification to at least diploma level in one of the following disciplines: social work, social care, psychology, counselling, youth work, community development and at least three years experience working in a Social Care setting.
